What type of forms does School Service, Inc. offer?
School Service provides a variety of over 250 forms for schools across the United States. From padded permission forms, passes, and absentees to perforated detention forms, 4-part purchase orders, 3-part progress reports, 2-part admission slips and emergency cards. We also provide strip tickets, lunch and milk punch tickets, roll tickets, bus and parking forms, envelopes, and much more. Can School Service customize a form to my specific needs?
We can custom make most any form you may need! Most forms can be printed in a variety of different ways including but not limited to:
- Bond paper padded
- Carbonless sets (2-part, 3-part, and 4-part)
- Book forms
- Index cards
- Name imprinting and numbering are also available

Can you customize forms for businesses other than schools?
Yes, we are willing to customize forms for other businesses.

Our school does not use purchase orders. Do I still need a purchase order number to complete my order?
Yes. The order will not be complete without a purchase order number. If your school does not use purchase orders, you can simply type in your name, the date, or any numbers you wish to use. However, it is not required if you place the order over the phone, fax, or mail.

Always include name of school, street address (UPS will not deliver to a post office box) and zip code. Please indicate if the billing address is different than the shipping address.
Please include name and phone number of a person who may be contacted about your order.
Indicate order quantity in number of sheets, cards, labels, envelopes, tickets, etc. Always include form number, description, and color.
Minimum order on most items is 100 sheets (one pad) or 100 cards - unless otherwise indicated.
Do not send money with your order, as shipping charges must be determined and added. We will bill you after a complete shipment is made.

Can you give me an estimate on shipping cost?
Estimate shipping costs:
- $5.00 to $8.00 for orders under $70.00
- A minimum of %10 of total on orders $70.00 and greater.
*Shipping cost may be less or greater than this estimate due to the location of the school and the weight of your order.

How soon will I receive my order?
99% of all stock forms are shipped the same day if received before noon (CST). Special printing can take one to two weeks to process and ship.
All shipments are F.O.B Union Grove, WI via UPS or Parcel Post, whichever is most economical, otherwise instructed. UPS delivery times include 3 business days for the East Coast, 1-2 business days for the Midwest, and 4-5 business days for the West Coast.
We encourage you to order as soon as possible, especially during the busy summer months!

Can you send my order to a residential address?
To protect our customers, we restrict sending forms that track student activities to residential addresses, unless permitted by the school. Please contact us at 1-800-747-9549 for questions concerning your shipping or billing address.

What is your payment method?
We will mail an invoice to your billing address after your order has been shipped. Normal terms are net 30 days; however, we may approve reasonable requests for any payment extensions. Please pay in full including shipping.
