customize your form by:
- Designing a form to your exact
specifications
- Modifying an existing
form
- Printing a competitor’s form (at best
price)
- Printing a form you have already
designed
- Including your school name &
information on a form (see Imprinting

- Starting from scratch and
help design a new form for your school/company
Most forms can be
printed on your choice of paper, including (but not limited to):
- Bond paper padded (note pad paper - 20
lb. stock or equivalent)
- Card stock paper (67
lb. stock or equivalent)
- Index card stock (110 lb. stock or
equivalent)
- Carbonless/NCR sets (i.e. two-part,
three-part, four-part, five-part)
- Book forms
(perforated carbonless paper in a comb-bound book)
Price and
Delivery:
- Quoted prices in effect for 30
days.
- A one-time setup charge of $25 - $55 is
included for plate and typesetting.
- The setup fee
is waived if you order $100 or more on your new custom
form.
- We will beat our competitor's prices! Contact us for
additional information.
- Delivered quantities may
vary 10% over or under ordered quantity. Billing adjusted
accordingly.
- Please expect 1 - 2 weeks for
delivery.

There will never be a charge for a typesetting revision of your custom
form.
Once you have a custom form printed by School Service, we will offer
free
typesetting revisions for as long as you continue to purchase
it.

Custom Forms Instructions
Simply
download & print our Custom
Form Quote Request Checklist, fill out the appropriate information, and
fax the checklist with the sample of your custom form to (888)
747-9549.
OR
Simply
send a sample, layout, or the artwork of the form via mail, fax, or
email:

If you send
your form via email, please attach the form to a digital format (i.e. Microsoft
Word, Word Perfect, Microsoft Publisher, Adobe PDF, etc.). We will also waive
the one-time setup fee if the artwork of your custom form is already set up on a
digital format. Exceptions may apply, such as, if you wish to have us modify
your form but it is on a non-modifiable format (i.e. PDF). Do not worry if
you do not have the artwork. You can make up your own from scratch or modify an
existing form. Remember, School Service will also print our
competitors’ forms at the lowest cost!
Don't
forget to include:
- Type of form (bond
paper padded, carbonless sets, index card stock, book form,
etc.)
- Color of paper or type of carbonless sets
(2-part, 3-part, 4-part)
- Size of
form
- Quantity desired
- Contact information
After we
receive your sample and information, the Special Printing Department will send a
price quote on your custom form via fax or email. Please reply back to us that
you accept the pricing. After your response, we will send a proof of the artwork
for your approval. Please review the form carefully and make any necessary
changes. School Service prints custom forms to your specific needs! Once we
receive your approval on the artwork, your order will be produced & shipped
within 1 – 2 business days (it may take 1 – 2 weeks during the busy summer
months or depending on the type of custom form).
