When placing your order online: School Service NOW offers an easier way to order! We added "school imprint" products (i.e. 45G-Imprint - Deficiency Report w/School Imprint) to most of our custom imprint forms. These forms include imprinting instructions & includes the imprint fee on your total order! Simply scroll down to the bottom to order your imprinted form on this very page! Please remember to indicate EXACTLY what you would like imprinted on your form in the Special Instructions Box of the Shopping Cart. Not including this information may cause delays on your order. Please contact us for imprinting prices and instructions for forms not shown below.

School Service can include numbering and imprint your school name, address (optional), & phone number (optional) for your purchase orders. We NOW offer a set price for our purchase orders with imprint & numbering. There is no longer a seperate imprinting fee and numbering fee when you order your purchase order. Please review the new prices.
